Creating Individual MS Forms

With Microsoft Forms, you can create surveys, quizzes, and polls, and easily see results as they come in.

Note: Only the individual that creates the form can view the responses. An individual MS Form is only suggested for use in instances where the data does not need to be shared widely.

Start a new form

  1. Sign in to 365.MyLMC.org with your LMC credentials.

  2. Select New Form.

  3. Title your form and, if you want, a description of it.

    Note: Form titles can have up to 90 characters. Descriptions can have up to 1,000 characters.

    Note: Your form is saved automatically while you create it.

Add questions

  1. Select  Add new to add a new question to your form.

  2. Choose what kind of question you want to add, such as Choice, Text, Rating, or Date question types. Select More question types  for Ranking, Likert, File upload, or Net Promoter Score® question types. To organize sections for your questions, select Section.

    Tip: You can also format your text. Highlight a word or words in your title or questions, and then choose any of the following: Bold (keyboard shortcut - CTRL/Cmd+B), Italic (keyboard shortcut - CTRL/Cmd+I), Underline (keyboard shortcut - CTRL/Cmd+U), Font colorFont sizeNumbering, or Bullets.

    Important: If you delete a question, it will be permanently deleted along with any response data that's been collected for it. Learn more.

Preview your form

  1. Select Preview to see how your form will look on a Computer or Mobile device.

  2. To test out your form, answer the questions in Preview mode, and then select Submit.

  3. To keep editing your form, select Back.

Form Settings

  1. Who can fill out this form?

    1. Anyone can respond - Anyone with the link can respond to the form

    2. Only People in my Organization can Respond - Only individuals with an LMC login can respond. You can choose to record the name of the responder and limit the person to a single response.

    3. Specific People in my Organization can Respond - You choose who can respond. You can choose to record the name of the responder and limit the person to a single response.

    1. Allow Receipt of Responses after submission

    2. Get email notification of each response.
